Patients must be established as patients of record to participate. New patients may enroll in the membership plan after completing their first visit and comprehensive exam. Membership benefits cannot be combined with any other promotional offers. Plan savings apply only to general dental services and do not include orthodontic procedures, implant surgery, Cone Beam CT, or panoramic X-rays.

Membership clubs are not insurance but a payment arrangement provided by Complete Dental Health for services rendered. Memberships are provided exclusively to uninsured patients of our practice and shall not be considered a pre-payment for future services or payment for access to discounted services. Rather our membership is a monthly plan that provides members access to reduced fees for services. Membership fees are non-refundable.
